By the end of this course, you will be able to:

  1. Recognise the risk of harm to patients, the magnitude of medical errors, and why patient safety is important.
  2. Describe the Science of Safety/System Design, Root Cause Analysis, and Just Culture/Accountability and their importance to reducing patient harm.
  3. Explain what an RCA is and when it should be completed.

This course takes approximately one and a half to two (1.5 – 2) hours to complete. You can do a self-paced-learning and interrupt; however, you will need to complete all activities to complete the course.

The target audience will be Root Cause Analysis (RCA) participants—any staff members at the facility, such as doctors, nurses, pharmacists, technicians, and support services.

It is preferred, but not required, to complete Healthcare Quality & Patient Safety Standards course.
